Membership Intake Form
Membership Intake Policy Certification Checklist
1. For each quarter your chapter intends to have membership intake, you must submit a
completed Membership Intake Intent Form and submit to the Program Coordinator for
Fraternity and Sorority Life at least 2 weeks prior to beginning the intake process.
2. Within the same packet, complete the Hazing Policy Compliance Certification and Hazing Compliance
Form with members’ signatures and submit to the Program Coordinator for Fraternity
and Sorority Life at least 2 weeks prior to beginning the intake process.
3. Once your new members have been initiated, you must update your roster online at
www.uc.edu/greeklife and submit the Member Registration Form to the Program
Coordinator for Fraternity and Sorority Life within a week after initiation.

Event Registration Form and Program Evaluation
All NPHC Chapters are required to submit program event registration forms for every event on the NPHC calendar. Forms are due no later than seven (7) days prior to the scheduled event. Failure to submit registration form may result in judicial procedures against the sponsoring chapters(s).

General Forms

Chapter Officer Information Form
This form is due immediately after there is a change in the chapter officer roster and/or when new officers are elected to the chapter.

Event Registration Form
IFC and Panhellenic chapter events are to be registered using this form. Event registration forms and neighbor letters are due five days prior to the event. Guest lists are to be submitted by 4:00 PM the day of the event or by 5:00 PM the day before the event.

Incident Form
This form is to be used to report violations of the risk management policy or submit a formal complaint against a fraternity/sorority to be handled by the Interfraternity Council and/or Panhellenic Council Judicial Process.

Member Registration Form
This form is for students who have participated in the fraternity/sorority recruitment process and have accepted an invitation (bid) to join a fraternity or sorority. This form is due immediately after an individual has accepted his/her invitation to join a fraternity or sorority.

Quarterly Report
This report form is due by 5 PM on the Friday before Finals Week. Information submitted to the Fraternity and Sorority Life Office will be compiled into a cumulative report.The long awaited End of the Quarter Checklist/Report is here!  

The new report is divided into two parts on the Excel sheet:
Part I (first tab):  Directions:  Fill in the point values for the items you have completed.  These items are the EXACT items from the Chapter Excellence Awards Application, just divided up into the quarter when your chapter should be completing them. 
 
The items are color-coded as follows:
Items to be completed during the spring 2007 quarter are purple.
Items to be completed during the fall 2007 quarter are red.
Items to be completed during the winter 2008 quarter are blue.
Items that are to be completed every quarter or any quarter are green.
 
**There is no need to provide me the necessary documentation for each item at this point. However, you should be organizing your Application binder throughout the quarter with this documentation so you stay on task.**

Please let me know if you have any questions, comments, or concerns.  I believe this will be a simpler process and will help keep your chapter on task in completing the Chapter Excellence Award Applications in a timely manner.  The complete 2007-2008 Chapter Excellence Award Application is also available above under Chapter Excellence Awards (in case you need explanations for the items in the checklist).
